Students that are exempt from the Bacterial Meningitis Vaccination, per Senate Bill 1107, need to fill out the Bacterial Meningitis Acknowledgement Form.

Students that are required by Senate Bill 1107 to submit evidence of immunization against bacterial meningitis, will need to submit the Bacterial Meningitis Vaccination_Acknowledgement Form.

Please return the appropriate form to the Office of the Registrar or to the Student Affairs Office in your School or College.
