Animal Care and Use Forms
The Institutional Animal Care and Use Committee (IACUC) is committed to making the approval process as trouble-free as possible. Below are the most current forms. Contact the IACUC office at animalcompliance@tamu.edu or 979.845.1828 with any questions. All forms are in Microsoft Word format and should be submitted electronically when possible.

• Animal Use Protocol
  • Note: Attachment 4 Use of Privately Owned Animals requires that the Owner Consent form be included in the AUP submission. There are two options for the Owner Consent form.

        i.            If you are a College of Veterinary Medicine & Biomedical Sciences investigators/instructors, use the Clinical Review Research Committee (CRRC) version for Owner Consent forms. You will need to submit these forms with your Animal Use Protocol to both the IACUC and CRRC.

       ii.            For all other investigators and instructors, please use the IACUC Owner Consent form.

• An AUP is approved for a three year period.
• AUP Amendment
• Animal Transfer
• Continuing Review is required annually on all protocols utilizing USDA regulated animals.
• Final Report
