Regional Healthcare Partnership 17
Regional Healthcare Partnership 17 (RHP 17) is a nine-county partnership,
formed on March 14, 2012, located in the eastern portion of the Central
Texas region. The partners consist of Brazos, Burleson,Grimes, Leon,
Madison, Montgomery, Robertson, Walker and Washington counties.
The Texas A&M Health Science Center (TAMHSC) has been appointed
as the RHP 17 anchor institution, with the Brazos Valley Council
of Governments (BVCOG) acting as the designated fiscal agent for RHP 17.

The Region 17 Anchor Team is pleased to release the full revised version of the RHP 17 Plan that was submitted to HHSC on January 25, 2013 in response to HHSC's technical feedback and forwarded to CMS on February 15, 2013. Over the next 45 days, CMS will conduct their review and work directly with the Anchor Team and Performing Providers should any revisions or additional information be needed. Following CMS review and revision, the RHP 17 Plan will be formally approved and enacted. Should the plan require additional changes, this website will be updated throughout the review process and the most recent version of the plan documents will be posted (last updated 02/21/2013).

DSRIP Project Proposal Portfolios
Portfolios containing PDF copies of each DSRIP Project Proposal submitted by a Performing Provider in the Region.
NOTE: Proposals were requested/collected from those entities designated to be Performing Providers as defined by HHSC. Based on current HHSC guidelines governing projects and DSRIP funding flow, any other providers participating must do so in a subcontractor role and are instructed to work directly with a Performing Provider for inclusion in projects.

COMMUNITY NEEDS ASSESSMENT INFORMATION
County Executive Summaries
The following are executive summaries for each county related to the community need assessments done for that area. In addition, the full 2010 Brazos Valley Assessment with county-by-county supplemental information (covering all but Walker and Montgomery Counties) that was compiled by the Center for Community Health Development of the Texas A&M Health Science Center School of Rural Public Health is included, along with the full 2011 Montgomery County assessment compiled and provided courtesy of the Montgomery County United Way. The Walker County Executive Summary was provided by Huntsville Memorial Hospital in conjunction with the Walker County Hospital District.
DSHS In-Depth County Profiles
The following are in-depth county profiles compiled for and provided to RHP 17 by the Texas Department of State Health Services (DSHS) through the Office of the State Epidemiologist. DSHS notes that a minimum of 150 hospitalizations over the five-year period measured were required within a county for an in-depth profile to be produced for any of the ten potentially preventable hospitalization conditions.
This information is made available to regions as a supplement to their own county health surveys and other needs assessment data. Executive Summaries and associated demographics, county rankings, facility information, and summaries of potentially preventable admissions/readmissions for all of the counties in RHP 17 can be found below in the MEETING MATERIALS for each individual county planning meeting held in May/June 2012.
ELECTRONIC WORKBOOKS for IGT REVIEW
The following are the electronic workbooks HHSC created and had each Performing Provider receiving DSRIP and/or UC payments complete and return to the Anchor. All funding discussions related to IGT and project/UC support took place between performing providers and IGT entities directly. The RHP 17 Anchor Team had no part in those discussions or negotiations. These workbooks are simply being provided here for convenience and as a service to those entities wanting to perform IGT reviews.
